License Application for Tourism Enterprise (Form 1)

The Ministry of Tourism offers an application for licence, or variation of licence to carry on a regulated tourist enterprise to persons keen to venture into such industry.
The applicants complete the form 1 with the name of the regulated tourist enterprise, which if is a limited company, state country and year of registration, address of principal premise and names of proprietors or directors for a limited company. Provide their postal and residential address, age, country of birth and current nationality.
State whether the enterprise will be a hunting safari and provide names of all professional hunters, and exact regulated tourist enterprise the venture will undertake in accordance with the licence. In addition, provide the names and addresses of all banks the enterprise will keep its money.

Application for a Death Certificate (Form D4)

The department of Civil Registration provides an application form for the issuance of death certificate.
The document has two sections, one for the applicant to fill out and the other for the officer of the department of civil registration to fill out.
The applicant should complete the form with the province and district that the death occurred, burial permit number, exact place of death, name of deceased, sex, date, age, occupation and the name and address of applicant.

Application for Late Registration of Death (Form 6)

In pursuant to section 8 of the Births and Deaths Registration Act, the Department of Civil Registration offers a form for late registration of death that occurs within Kenya.
The application is done through the Registrar of Deaths. With a prescription fee and receipt of proof, the applicant state the name of the deceased, place of birth and the date of death.
The Registrar following the evidence provided by the applicant in support of the information given therein completes the other part.

Registration of Death of a Kenyan Citizen ocurred Abroad

This form is an application form for registration of death of a citizen of Kenya occurring abroad.
The applicant must provide all the information concerning the deceased. These include the full names of the deceased, age, sex, occupation, exact place and country of death, deceased normal residence in Kenya.
If a medical practitioner certifies death, they should state cause of death, and provide their name and address. If not, the applicant, states cause of death and certify as the informant that information given is correct to the best of their knowledge.

Application for Registration of Cooperative Society (Form 1)

The Cooperative Societies Act provide a way for application for registration of Co-operative Society.
The Commissioner of Co-operative Development receives the completed form from applicants who have successfully filled out the application form with names of proposed Co-operative Society, area of operation, type of Society, limited or unlimited liability, language of books and accounts, name of secretary and the registered office and postal address.
The officials of the Society should then sign the application form, providing their names, occupation, age, address and signatures.

Register of Death (For use by next-of-kin where no medical certificate of cause of death issue)

This document is use by the next of kin where there is no medical certificate to show the cause of death.
The applicant gives the deceased name in full, date of death, age, sex, occupation, exact place of death, normal residence of the deceased.
The next part, the applicant certify by placing a tick in the box against the description, which most describe the condition before death. Options fall either on natural or unnatural causes. Then committing by certifying they are the relative or next of kin or the capacity in which the information is given.

This document is for use by medical practitioners and in hospitals.
The applicant state their district, the full names of the deceased, date of death, sex, age, exact place of death, occupation, and the normal residence of the deceased.
The medical officer, who must give the cause of death and certify all the examination done to the deceased, completes the second part.
