Application for Registration as a Citizen of Kenya by a Stateless Person (Form 12)

This document provides an a avenue for application to be a Kenyan citizen by registration for a stateless person under section 15 of the Kenya Citizenship and Immigration Act of 2011.
Fill the form in block letters , giving the names, particulars of original name if changed, place and date of birth, nationality at birth and the circumstances for which it was lost.
Provide current contacts, occupation and marital status and if married, give place and date of marriage, name and nationality of spouse.
Give the father’s and mother’s full name, address and nationality. If deceased, state nationality at time of death. Provide also the particulars of children if any and current and previous residence in Kenya.

Notice of Cessation of Business Name

The Registration of Business Names Act Cap.499 section 15 provides for a notice of cessation of business for businesses ceasing to operate.
Addressed to the Registrar- General.
This notice allows the applicant while stating the business name and number to apply for cessation of the said business with effect from a particular date and year, save for the purpose of winding up the said business.
The notice must be signed by the proprietor or all partners, or a Director.

Notification of NGO Registered Office (Form 4)

This document is offered to the Non- Governmental organizations looking to change the location for their registered office or contact address.
It is directed to the Executive Director of the Non-governmental Organisations Co-ordination Board.
The desired organization gives notice to the board and fills the Form 4 with details of where it will have its registered office, where it is moving from and to, where it will have its postal address and if it will change its postal address.

NGO Register Form (Form 1)

Organizations requiring registrations has Non-governmental entities are required to fill this form.

NGO Non-citizen Work Permit Endorsement Form

This is filled by non citizen working with NGO to enable them work in the country.
With this 5 page document, the organisation is required to provide alongside its name and both postal and physical addresses, telephone numbers, the name of the non-citizen employee seeking the work permit, their job title, residential address, passport number, nationality, sex, age, level of education, profession, contract duration, application type whether new or renewal.
It should further provide category of contract, annual salary and other benefits. This should also include the reasons for seeking the expatriate and what specific job requirements for the expatriate position.
The organisation also fills in the document if it has offered an understudy for the expatriate, and gives their full names and position as well as academic and professional qualifications of the understudy.

Police Absract Form (Appendix 4)

The Kenya Police Service have been providing the abstract form to Kenyans for reporting incidences of lost or damaged important documents like the national Identification Card, Passport, ATM cards and other important documents.
The applicant completes the form with their names, address, and stating the lost or damaged document. The Officer in charge complete their part and stamps the document.
In most incidences the completed abstract form stands in for the lost or damaged document when transacting business.

Compliants Against the Police

In line with the Police reforms, the Kenya Police Service avails a form for complaints against a police Officer by any Kenyan citizen.
The applicant must state which station their complain falls, where and when the incident happened and a brief description of what transpired.
Provide contact details of any witness, and any details of the Police Officer involved In the incident together with the complainant’s contacts.

Medical Examination Form (Form P3)

The famous and largely infamous form in Kenya could be the Police P3 form. It is liked by many while at the same time dreaded by many who find themselves in confrontation with the law one way or the other.
The first part of the form is completed by a Police officer requesting examination from a medical doctor in favour of the victim, or aggrieved party. They provide their names, age, address, date and time of the alleged offence, giving a brief details of the alleged offence.
Part two, section A of the form is completed by a medical doctor after examining the victim. They state the conditions of the clothing, give general medical history and the general physical examination.
The B section is completed after section A, and in all cases of assault, including sexual assault. It entails the site, situation, shape and depth of the injury, age of injury and the probable assault weapon used as well as any treatment administered before examination and the degree of harm.
Section C is completed in alleged cases of sexual offences, and after Sections A and B. it highlights the nature of offence and the estimated age of the person examined. It is divided into two parts; male and female complainant and details of injuries are outlined here.
Section D is set out for the male offender. The man accused of any sexual offence. The descriptions of extent of their heinous act is penned on these slots.
After additional remarks by the doctor, he/she append their name, signature and date.
